Job Details
Maxim Healthcare is looking for a GP for a long term assigment in the south Yorkshire Area this is to start in August.
If you are interested in providing long term cover to this surgery.
The applicant will need:
· Fully up to date CV with 2 referees
· Full GMC registration
· JCPTGP / PMETB certificate
· Full Medical Insurance
· Primary Care Trust Listing letter (PCT Listing)
· Passport showing the Photograph Page
· Immunization report within the last 5 years showing your Hepatitis B
· Enhanced CRB
